WE CANNOT ACCEPT YOUR REPAIR UNLESS THIS FORM IS COMPLETED AND INCLUDED IN YOUR PACKAGE. (After completion of this RMA form, a copy will be emailed to you, to include in your package. Please allow 12-24 hours to receive your email).
VIAVI meters will require a $350 evaluation fee if the repair is rejected by the customer and returned to the customer. This fee is waived if the repair is approved.
VeEX meters will require a $200 evaluation fee if the repair is rejected by the customer and returned to the customer. This fee is waived if the repair is approved.
ALL SHIPPING COSTS FOR NON WARRANTY REPAIRS WILL BE INCURRED BY THE CUSTOMER.
