Terms and Conditions
- Credit Extension: Destiny Solutions Inc., located in Georgetown, Indiana, may extend credit to qualified applicants at its sole discretion.
- Required Information: All fields in this application must be completed accurately and in full. Incomplete applications will not be processed.
- Identification and Payment Verification: Applicants must provide:
- Clear copies of the front and back of a government-issued photo ID.
- Clear copies of the front and back of the credit card intended for payment.
- Credit Authorization: By submitting this application, you authorize Destiny Solutions Inc. to charge the provided credit card for any outstanding balances 30 days after shipment.
- Failure to pay within 40 days of shipment constitutes a default and a violation of this agreement. This includes any decline of payments once we try to run your card on file.
- Personal Guarantee: The individual signing this application personally guarantees payment of all debts incurred, regardless of the applicant company's ability to pay.
- Ownership: All products and/or equipment purchased on credit remain the property of Destiny Solutions Inc. until paid in full.
- Credit Information Release: You authorize Destiny Solutions Inc. to obtain credit information from financial institutions and trade references listed in this application.
